Request Type logging procedures

The steps to log a request are different, depending on the Request Type selection:

Standard

Any request that does not fall into the other categories.

BOC

Business Office Client

Continuity of Care

Generally refers to the sharing of information about a patient between multiple health care providers. For example, an allergy specialist may need to see the medical records from a patient’s primary care physician.

May also include requests made when the patient is forced to switch providers because of a change in insurance coverage.

Patient

Patients (including parents or guardians) can request copies of their own medical records.

DDS (Disability)

Social Security Administration (SSA) offices request records to determine a person’s eligibility for disability benefits.

Pull List

Some requesters send a single Request Letter that asks for the records of multiple patients. The patients are typically listed in a spreadsheet-style document, called a Pull List.

HealthSource Clarity normally identifies Pull Lists automatically, but they can be manually logged if the automatic process does not work properly.
